Liability Waiver & Release
I acknowledge that participation in [Club Name] activities involves inherent risks of physical injury and I agree to assume all such risks. I hereby release, waive, and forever discharge the Buffalo Racquet Club, its owners, Board of Directors, employees, and agents from any and all liability, claims, or demands for personal injury, property damage, or wrongful death.

Parent/Guardian Consent: If I am purchasing this membership for, or allowing its use by, a minor under the age of 18, I certify that I am the parent or legal guardian of that minor. I expressly grant permission for the minor to participate, and I agree to bind myself, the minor, and our executors to all terms, liability waivers, and indemnifications contained in this agreement.
